Dr. Chan received his medical degree from the University of Toronto and his residency in Family Medicine at the Carilon Clinic in Roanoke Virginia.
He has held past appointments as an Affiliate Assistant Professor at the University of South Florida College of Medicine. Dr. Chan’s medical interests focus on integrating the knowledge and traditions of multiple medical disciplines to help optimize his patients' health throughout their lifespan.

Karen Johnson, APRN
Karen Johnson, APRN, has been helping patients obtain and improve their quality of life for over 20 years. She received her undergraduate degree in psychology and health, physical education and recreation from Mercer University and her M.S. in physical education and exercise science from Michigan State. She also has extensive training in weight loss and bioidentical hormone therapies.
Karen is Board Certified in Family Practice and is an APRN Preceptor for the University of South Florida School of Nursing.
As an exercise and healthy living advocate, Karen motivates others to adopt active lifestyles. She uses a combination of traditional medicine, bioidentical hormones, and supplements to provide balanced care plans.
Therese Grandlienard, APRN
Therese is a compassionate patient advocate providing dignified patient care. Her 12+ years experience in nursing, and energetic attitude makes her a great partner for your family’s health and wellness.
Therese’s professional experience includes service at James A Haley Veterans Hospital, Life Scan Wellness Centers, and Sand Bay Primary and Urgent Care. She is a licensed APRN and has Multstate RN licensure.
